United Opticare cannot warrant that the information is accurate, complete, up to date, free of infection by viruses, worms, Trojan horses or anything else manifesting contaminating or destructive properties, legal, legitimate, valid, correct and reliable. You acknowledge and agree that data collected and maintained by United Opticare with regard to its users may be disclosed in accordance with United Opticare's Privacy Policy and for the reasons set out below. Users agree to allow United Opticare to track, record and maintain statistics related to visitors on its websites, including but not limited to, the source of users' entry to the site(s) and navigation throughout the site. Users agree that submissions and users' e-mail addresses may be used by United Opticare for the purpose of contacting them for marketing its sites or other purposes. You expressly acknowledge and agree that United Opticare may preserve content and may also disclose content if required to do so by law or in the good faith belief that such preservation or disclosure is reasonably necessary to comply with legal process, obtain references or information on the suitability of a locum, to allow United Opticare to undertake its work as an agency, enforce the Terms and Conditions, respond to claims that any content violates the rights of third-parties and/or protect the rights, property, or personal safety of United Opticare, its users and the public. You acknowledge and agree that the technical processing and transmission of your content may involve transmissions over various networks and/or changes to conform and adapt to technical requirements of connecting networks or devices. Users must either be registered pharmacists who are willing and available to undertaken locum work via United Opticare or registered pharmacies requiring locum cover via United Opticare. The use of the web site outside these criteria is strictly forbidden and may result in legal action. Disclaimer The use of the website is at your own risk. The products and services are provided 'as is'. United Opticare disclaims any implied and express warranties, statutory or otherwise, guarantees and/or liabilities as to merchantability, fitness for a particular purpose, accuracy of content, non-infringement of proprietary rights, freedom from computer virus, condition, quality, durability, performance, uninterruption, timeliness, secureness, error freeness and/or reliability of products and services, the website and/or information accessed, to the fullest extent permitted by law. Locum Specific Terms & Conditions Registering with United Opticare shall be deemed to be acceptance of these Conditions. The locum hereby agrees to notify United Opticare of the occurrence of any of the following events:
The client following completion of each booking will make payment directly
to the locum.
United Opticare acts only as an introduction service to the client and the locum is usually responsible for his or her own Income Tax and National Insurance contributions which are assessable under Schedule D of the Finance Act. If the locum, or any third party, suffers any losses either financial or otherwise or suffers in any other way whatsoever due to any act or omission of the client then United Opticare is in no way liable. All information supplied by United Opticare to the locum is done so in good faith and United Opticare is in no way whatsoever responsible for any inaccuracy or omissions whether these are due to some act or omission by United Opticare or other third party or parties. The locum shall not use information received from United Opticare to directly book locum work with the company, without informing United Opticare, or to recruit or assist other locums in employment or for any other purpose not directly linked to their locum work via United Opticare. The locum shall divulge no business information or materials, either supplied by United Opticare or the client to any other party or use this information for any other purposes than solely for the securing of locum work via United Opticare. Failure to abide by the terms and conditions agreed to when registering may result in a complaint being made to the General Optical Council. The locum agrees to attend his or her placement procured or obtained by United Opticare dressed in manner as befits their professional status and always to abide by the Pharmaceutical Society Code of Practice. The locum will be subject to and abide by all health and safety at work regulations and company security rules. Locums should inform United Opticare of any investigation by the police or General Optical Council or any other investigation, which has or may have a bearing on their continuing membership of the General Optical Council, or their fitness to practice. Locums should ensure they have undertaken all necessary training to allow them to provide the services offered by the pharmacy, which will include repeat dispensing. The locum is required to attend all placements booked via United Opticare. A booking for work will be considered as confirmed with the locum when the locum has accepted the offer of work, whether via the United Opticare computer booking system, verbally or in writing and United Opticare has confirmed the booking with the client. The booking cannot be cancelled except with the agreement of United Opticare. The locum may be asked to vary a booking to assist a client or United Opticare as long as this is reasonable. The locum must arrive at the branch where he or she is booked to work at the allocated time. The locum will only be paid from the time he or she arrives at the branch. If the locum is late, he or she must contact the branch and advise them of his or her estimated time of arrival. Where a client with whom the locum was booked to work cancels a booking once confirmed United Opticare will use its best endeavours to obtain alternative locum work. If the locum suffers any financial loss or any other loss whatsoever as a result of the clients actions, the client and not United Opticare, would be liable for such loss if appropriate. Should the locum feel that he or she is unable to attend work due to illness, the locum must advise United Opticare of the situation and telephone the branch where he or she was due to work. The locum will be required to arrange for an alternative locum. United Opticare is prepared to offer a referral fee of upto £150 for each locum that contacts me via your recommendation. The only conditions are: The locum introduced must book and work 40 locum days via United Opticare and United Medicare Limited must receive all the booking fees for this work before you receive payment. A lower rate of £30 is payable for 12 locum days if they are unlikely to work 40 – the choice is yours but it can only be one or the other. They must not have already worked for United Opticare (or subsidiaries) or on a direct booking basis for the companies in the area they are booked or via another agency for the companies in the area they work such that the companies would not be prepared to accept them as a United Opticare locum and therefore not pay an agency fee to United Opticare. Remember, the finder’s bonus is only payable upon United Opticare receiving the booking fees from the companies. (This generally is a problem only for locums who work in their local area and regularly work for these companies either directly or booked via an agency rather than locums from other parts of the country). The locum must contact United Opticare and state that you introduced them. The offer is valid until further notice and may be withdrawn at any time without notice. Company Specific Terms & Conditions These terms and conditions shall form part of every contract between United Medicare Limited trading as United Opticare (hereinafter United Opticare) and any person, firm or company (hereinafter referred to as 'the client') that uses the services of United Opticare to procure the services of a locum pharmacist (hereinafter referred to as 'the locum'). Upon using the services of United Opticare, the client shall be deemed to accept these standard terms and conditions. United Opticare reserves the right upon giving fourteen days written notice to vary these standard terms and conditions. 1.
LOCUM REMUNERATION The client shall be responsible for paying remuneration directly to the locum forthwith upon termination of the engagement or weekly, whichever is the lesser period, unless the client and the locum mutually agree some alternative schedule. The locum and the client shall share sole responsibility for ensuring the payment of all and any Income Tax, National Insurance Contributions and other statutory payments in accordance with Schedule D of the Finance Act from time to time. 2.
BOOKING FEE The client accepts and agrees that United Opticare acts only as an introduction service between the locum and the client and the client accepts and agrees that United Opticare shall not be liable to the client for any losses or costs of any nature howsoever or whatsoever should the locum be unable to fulfil a booking that he or she has accepted. In the event a locum in unable to fulfil a booking, United Opticare shall use its best endeavours to introduce a suitable replacement and, failing that, will either refund or not levy a booking fee as appropriate. They are subject to change as stipulated herein and we may from time to time negotiate with you to mutually agree particular booking fees for a specific booking. Booking fees (standard or particular) shall remain in force until altered by United Opticare upon giving the client fourteen days written notice of change. If
the client (or any associate of the client) within twelve months of
the client's engagement of a locum booked through United Opticare
re-engages under any circumstances and under any form of contract
said locum, the client forthwith must inform United Opticare, and:
- The client for a period of twelve months from its termination of the engagement of a locum booked through United Opticare shall not without first informing and obtaining the consent of United Opticare introduce said locum as a locum or as a prospective employee to any third party. If the client within a period of twelve months from its termination of the engagement of a locum booked through United Opticare with or without the consent of United Opticare introduces said locum as a locum or as a prospective employee to any third party, the client shall immediately become liable to United Opticare for the United Opticare booking fees from time to time. If within 5 days of a locum being engaged by the client or through the introduction of the client in the circumstances set out in the preceding two paragraphs the client fails to notify United Opticare, the client (in addition to any applicable booking or buy out fees from time to time) shall immediately become liable to United Opticare for a default payment calculated at 30 times the daily booking fee from time to time. 3.

The client accepts and agrees that United Opticare cannot be held responsible or liable for any acts or omissions of any locum booked through United Opticare. The client accepts and agrees that United Opticare cannot be held responsible or liable for:
All locums booked through United Opticare to a client will be currently registered with the Royal pharmaceutical Society of Great Britain. All clients must ensure that they have suitable professional indemnity insurance in force that covers the period of engagement of the locum and the professional services and other services the locum supplies. The client shall not without the express written consent of United Opticare share with any other parties any business information or materials or data supplied by United Opticare. All documentation and information and material provided by United Opticare is the intellectual property of United Opticare and may not be copied, reproduced, stored in a retrieval system or transmitted in any form or by any other means, either in part or full, without the express prior written consent of United Opticare.
